Firefighter Application Instructions
How to Apply
A City of Fitchburg designated application form is required for all positions an individual is interested in applying for. Resumes will not be accepted in lieu of a completed application form. Applications are accepted daily from 7:30 A.M. to 4:30 P.M., Monday through Friday. Contact Human Resources at (608) 270-4211 if you have specific concerns or questions about the recruitment.
Deadlines (as stated on the position announcement)
Completed application materials must be RECEIVED by the date specified in the job announcement. You may mail or drop off your completed application materials at the City of Fitchburg (City Hall Building), 5520 Lacy Road, Fitchburg, WI 53711. Applications received via fax or e-mail will NOT be accepted. Failure to submit all required application materials by the deadline indicated will disqualify you from consideration. Please note the list of items below, which outlines what to include when returning your application materials for consideration.
Complete Application Materials Will Include the Following
- Application For Employment - please fill in all areas - DO NOT indicate, “see resume”, should your application become separated from the rest of the information, we would be unable to consider you further. Additionally, applications must include your signature on the last page. Failure to do so will disqualify you.
- Completed “Firefighter Candidate Education, Training, and Experience Questionnaire”, with all supporting documentation.
Firefighter Questionnaire - For Firefighters (Volunteer and Full-time)
- Completed “Applicant for Firefighter - Classification” form

- Completed “Authorization for Release of Information” forms, there are two (2).
Authorization for Release of Information
- Cover letter, Resume, and additional references (optional)
- Affirmative Action Supplement (optional).
