If you would like to be considered to serve on a Board, Committee, or Commission, please fill out the Mayoral Appointment Application and mail to 5520 Lacy Road, Fitchburg WI 53711 or e-mail the form (please be sure you have digitally signed it using Adobe Acrobat version) to the email address listed below to a specific vacancy/expiration committee you are interested in.

If you are interested in serving on an AdHoc committee or any available open committee you can fill our the form and mail to City Hall or email to marcie.rekowski@city.fitchburg.wi.us
