The City of Fitchburg Clerk's Office offers our citizens a wide variety of services from issuing licenses and permits, election administration and City Council packet information to retention of official City documents and public notifications.  The City of Fitchburg Clerk's Department is located on the second (main) floor of City Hall at 5520 Lacy Road, Fitchburg WI 53711 and operates with a staff including the City Clerk, the Deputy Clerk, and two Secretary/Receptionists.  Office hours are from 7:30 a.m. to 4:30 p.m. Monday through Friday.

 

Open Records Requests

Forms

The City of Fitchburg currently does not accept electronic submission of the forms found on the City Clerk's pages.  Downloaded forms must be completed, printed, signed, and returned to the Clerk's Office.
