Dog Licenses
You may apply for a Dog License in the Clerk's Department at City Hall. You may also mail the application and rabies information to City Hall at 5520 Lacy Road, Fitchburg WI 53711. We will then mail the license, tag, and your rabies information back to you. The charge for a dog that is spayed or neutered is $15.00. The charge for a dog that is NOT spayed or neutered is $20.00. Dogs not registered prior to April 1 will be assessed a $5.00 late fee.

Cat Licenses
You may apply for a Cat License in the Clerk's Department at City Hall. You may also mail the application and rabies information to City Hall at 5520 Lacy Road, Fitchburg WI 53711. We will then mail the license, tag, and your rabies information back to you. The charge for a cat that is spayed or neutered is $5.00. The charge for a cat that is NOT spayed or neutered is $7.00.

| Since we do not keep paper copies of rabies information on file, please bring (or mail) the document (or a copy) that you received from your veterinarian showing all rabies information. According to State Statute, we must see this information every year. |
Stats. 95.21 (10) PENALTIES. (a) Failure to obtain rabies vaccination. An owner who fails to have a dog vaccinated against rabies as required under sub. (2) (a) may be required to forfeit not less than $50.00 nor more than $100.00.
Stats. 174.05 (5) The City of Fitchburg, will assess a late fee of $5.00 from every owner of a dog 5 months of age of over, if the owner failed to obtain a license prior to April 1 of each year, or within 30 days of acquiring ownership of a licensable dog or if the owner failed to obtain a license on or before the dog reached licensable age.
95.21 (2) RABIES VACCINATION REQUIREMENT FOR DOGS. (a) The owner of a dog shall have the dog vaccinated against rabies by a veterinarian within 30 days after the dog reaches 4 months of age and revaccinated within one year after the initial vaccination . . . The owner of a dog shall have the dog revaccinated against rabies by a veterinarian before the date the immunization expires as stated on the certificate of vaccination or, if no date is specified, within 3 years after the previous vaccination. Cats: Evidence of rabies vaccination is required.
